Providers

Providers participating in the Accountable Care Coalition of Direct Contracting, a Global and Professional Direct Contracting Model Entity (DCE), are on the forefront in the movement to reward coordinated, high-quality care.

As a provider, you will have the opportunity to comprehensively care for your Medicare patients with the support of programs to help improve beneficiary engagement and lower costs, such as greater access to skilled nursing facilities, telehealth services, and beneficiary engagement incentives.

Accountable Care Coalition of Direct Contracting partners with Collaborative Health Systems (CHS), an organization dedicated to enabling providers to operate in a quality-focused, value-based model.
